POSITION:                             HEALTH RECORDS OFFICER

 

REPORTING TO:                  SUPERVISOR - CASE MANAGEMENT

 

DIRECTLY SUPERVISES:    None

 

 

PURPOSE OF THE POSITION:

 

To ensure efficiency in records management and quality medical files underwriting for the provision of quality medical service delivery to all our clients.

 

KEY PERFORMANCE AREAS:

 

  1. Ability to provide stellar customer service to clients.
  2. Accuracy in assessment, interpretation, and intervention on the various cases.
  3. Maintain a customer satisfaction index of over 90%
  4. Effective management of hospitalization costs
  5. Accuracy and timeliness in providing progress reports on new and old cases
  6. Documentation and storage of clinical data.

 

DUTIES INCLUDE:   

       

  1. Creating new medical records and retrieving existing medical records 
  2. Assigning and recording new record numbers
  3. Verifying existing record numbers
  4. Responsible for accurately organizing patients' medical histories.
  5. Compiling, processing, and maintaining medical records of patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system by gathering appropriate record folders and contents;
  6. Preparation of daily/quarterly/monthly inpatient illness report.
  7. Prepare providers' surveillance reports.
  8. Displaying any significant medical problems prominently and communicating contentious issues with respective Care Manager 
  9. Discharging all functions towards improving the quality of the service by ensuring easy accessibility of the patients' records.
  10. To provide quality customer care to all stakeholders
  11. To prepare daily case management reports for discussion and planning courses of action that favor the company as per the scheme rules and guidelines
  12. To prepare weekly reports on admissions with detailed analysis on the reason for admissions and plans to ensure non-recurrence through disease management programs

 

LEVEL OF EDUCATION & EXPERIENCE

 

  1. Diploma in Health Records and Information Management
  2. Registered with relevant regulatory Body
  3. Computer Literate
  4. At least one (1) year of working experience in a similar capacity.
  5. Database management skills and ability to use statistical software.

 

OTHER COMPETENCIES:

 

  1. Knowledge of medical terminologies, 
  2. Solid understanding of medical privacy policies and principles.
  3. Proficiency in the usage of electronic health record technologies.
